I have received and read the Child Care Agreement and I agree to comply with the rules and responsibilities stated therein.

I agree to pay $ per week for these arrangements as well as any overtime charges of $10.00 per each 30 minutes, or portion thereof, if my child is not picked up by the time I have agreed to retrieve him or her.
I agree to deliver payment in advance by personal check, made out to Shohreh Katoozian, or by cash on Monday of each week and understand that my full week payment will also include any applicable holidays (listed in the Child Care Agreement) which occur during the week.
I understand, as it is stated in the Child Care Agreement, that my payment commitment is based upon the hours committed to each day, not the actual number of hours my child attends Sho Sho's Daycare. For example, I understand that I cannot pick my child up after only four hours on Monday and 'make up' for it by leaving him or her for thirteen hours on Tuesday and that any additional hours, beyond the number previously agreed (above), will be subject to overtime charges. Moreover, if my child is ill or on vacation and unable to attend daycare for any of the agreed days (above) I understand that in order to hold my child's place in SHo Sho's Daycare, full weekly payment is still due.
